I Know Exactly What I Am Doing

Ciara could teach this. She has, in fact, taught this.

She is a nurse of nineteen years and she has stood in front of a room of second years with a slide behind her and explained tolerance, and dependence, and the difference between the two, and why the second one is not a moral event. She was good at it. Somebody told her afterward it was the clearest they had heard it put.

She drove home from that session and did the thing she does.

There is no gap in her understanding. That is what makes this so strange to sit inside. She is not a person who was fooled by a molecule she did not understand. She can name the receptor. She can describe her own withdrawal curve better than most prescribers could. She knows, to the hour, what is happening in her body at any given point in a day, and she has known throughout.

And knowing has not slowed it by one week.

That is the thing she has never said out loud, and it is worse than the substance itself, because it undoes something she has quietly built her whole adult life on: that understanding a thing is most of the way to handling it.

I know precisely what is happening to me and it has made absolutely no difference. And if knowing does not help, then what exactly am I?

And here is where I stop talking about Ciara, because you know your own mechanism too. You may not be a nurse. You may simply be the sort of person who reads, who understands, who could explain their own situation better than anybody who has ever tried to help them.

Let me say the first thing carefully, and I want it clear of any sting: your understanding is not the failure here. It has not let you down and it is not a mockery. It has just been asked to do a job it was never able to do, and nobody told you that, and you have been reading the result as evidence about yourself.

Scripture describes this exact split

Let me put the true thing down first, because you have been feeling like an anomaly and you are not one.

You are not a strange case and this is not a private absurdity. The gap between knowing and doing is described in Scripture, in the first person, by somebody who was neither ignorant nor casual about it: And it is the Holy Spirit who makes that land in you rather than merely read well.

For that which I do I do not understand: for what I would, that I do not; but what I hate, that I do.Romans 7:15

Read who is speaking. That is Paul, who knew the law better than almost anybody alive, writing about the exact experience you have never said out loud: full knowledge, no traction. The Holy Spirit is the one carrying that into the room you are actually in.

And notice that the passage does not treat it as a failure of study. He does not conclude that he needs better information. The whole argument runs the other way: he ends the chapter looking away from himself entirely and toward a Person, and the next thing out of his mouth in the letter is that there is now no condemnation.

So the split you are living in is described, and named, and answered in Scripture rather than diagnosed as a defect in you, and God's Spirit is the one carrying that into a car on the way home from a lecture you gave. It is not the words doing that; it is His Spirit breathing through what God already said.

Knowledge cannot do the one thing

Now let me be exact about what has happened, because precision is what you respect and you have earned it.

Knowledge can identify a mechanism. It can predict a curve. It can tell you what will happen at hour thirty and it can tell you why. It is genuinely powerful and none of your training has been wasted.

What knowledge cannot do is want something different at eleven at night. It has no leverage there at all. That is not a defect in your education, it is a category boundary, and you have been standing on the wrong side of it applying the wrong instrument with great skill.

And there is a second thing your knowledge has quietly done, and this is the one to watch. It has kept you from asking, because a person who can explain their own situation feels absurd taking it to somebody who will explain it back to them.

Hear this: what you actually need is not information, because you have all of it, and no more of it is coming. What you need is help, which is a different substance, and needing it does not undo one thing you know. That is written, and the Holy Spirit is the one who makes it something you can act on rather than something you find intellectually acceptable.

And now the practical half, and I will be exact, because you will notice if I am not. Do not adjust, stop, taper or ration anything on your own, and your training is precisely why you must not: you are able to build a plausible plan and carry it out alone, and alone is the dangerous variable rather than the plan. Take it to a prescriber, and if you are in a profession where this is complicated, there are confidential services specifically for clinicians in exactly your position, and they exist because you are not rare. The sentence to say is not a diagnosis, because you could write one. It is: I know exactly what is happening and it has not helped, and I am asking for help.
